Austin Public Health is seeking residents of Travis, Williamson, and Bastrop counties to complete a Community-Wide Wellness and Preparedness Survey to better understand how COVID-19 has impacted basic needs access and mental health. The survey also asks about community perception of a future COVID-19 vaccine.
The survey is 5 questions and should only take several minutes to complete. It will close November 8, 2020 unless extended.
