The complainant alleges: This is a request for assistance in recovering damages for a auto
collision between an APD vehicle and my S10 Blazer. An unknown APD officer collided

result of the collision. Please direct me to the proper department or city representative who
can process such a claim. I would like internal affairs to investigate the anonymous cryptic
note, the incorrect information in the incident report, and why APD failed to follow up on
this collision.
This notice of complaint is a request for Internal Affairs to initiate an investigation to
determine if the employee conduct is within compliance of APD policy, Civil Service
Rules, and Municipal Civil Service Rules.
