In order to submit a registration application online you must be able to meet the following requirements. If you are unable to comply with the electronic submission requirements please print and complete the application found at the link below. Registration cards will be mailed to the address provided within 5 - 10 business days after payment is processed.
Online applicants must be able to pay by phone, using a credit card and must be able to submit the following electronically *
Government Issued Photo ID *
[Example: Texas Driver’s License]
Food Manager Card/Certificate *
Not required for replacement cards
Must be an exam certificate, not a prep course
Must be issued from a Texas Department of State Health Services approved program
Once the application is submitted, a confirmation number will appear on the screen. Please reference this number with your payment or when inquiring about your application.
