Join us for a community health public forum to learn more about steps being taken to increase well-being and improve quality of life.

Why: The forum will have an open house format with interactive areas where attendees can:
Review findings from the evaluation of the 2012-2016 Community Health Assessment (CHA) /Community Health Improvement Plan (CHIP) and help prioritize evaluation findings.
Review the 2017 CHA framework, learn about opportunities to provide feedback for the CHA, and help identify attendees for the next public forum for the CHA expected to occur in February.
